Dear Parent/Carer
COVID-19 Vaccinations – Second Dose
Thank you for letting us know that you wish your child to have their second dose of the COVID-19 vaccination in school.
The NHS Vaccinations Team is looking to carry out these vaccinations in school on Friday 28 January 2022 in the morning.
In order for your child to have this vaccination, please click here to submit your consent form by Tuesday 25 January 2022.
Please note, anyone who has tested positive for Covid-19 must wait a minimum of 12 weeks before receiving their second dose.
